This form is required when adding a player/coach to a
roster after the team roster has been approved by the district commissioner,
It is the form that everyone calls the "add/drop" form.. "After
completing the form and having it signed turn in the four pages to the
District Commissioner".
Four copies will print labeled white, yellow, pink, and
gold copy.
